Primarily responsible for evaluating concurrent documentation so that medical records appropriately reflect the severity of patients’ conditions. Under the limited supervision of the department manager and lead CDI specialist, will conduct reviews of medical records to identify missing, vague, and/or incomplete diagnoses; communicates the need for additional documentation to the appropriate medical and clinical staff; ensures the additional documentation is completed in a timely manner; works collaboratively other departments including Coding, Performance Improvement, Compliance, Medical Staff, Utilization Review/Case Management, Medical Records and any other applicable departments to improve the quality of physician documentation; supports initiatives to improve the quality of documentation by all caregivers.
Education and Experience
Bachelor’s degree in Nursing with at least five (5) years of related clinical experience required. Five (5) years of hospital clinical experience with nursing diploma or Associate’s degree would be considered in lieu of the educational requirements. Coding experience a plus.
Certificates, Licenses, Registrations; Current licensure as a Registered Nurse in state of Indiana. CDI certification preferred.
